Assumption of Risk and Waiver of Liability - Northern Lions Youth Soccer Club

INFORMED CONSENT AND ASSUMPTION OF RISK AGREEMENT
(For Participants 17 Years Old and Younger)

1. Binding Agreement

This is a binding legal agreement. Please review carefully before accepting. As a Participant in the sport of soccer, including training, practices, competitions, and related activities (collectively, the “Activities”), the undersigned, being the Participant and the Parent/Guardian of the Participant (collectively, the “Parties”), acknowledge and agree to the following terms.

2. Disclaimer

The Northern Lions Youth Soccer Club, its directors, officers, employees, coaches, volunteers, and agents (collectively, the “Club”) are not responsible for any injury, personal injury, property damage, expense, or loss of any kind suffered by a Participant during or as a result of the Activities, caused by the risks, dangers, and hazards associated with participation in the Activities, including those arising from the negligence of the Club.

3. Description of Risks

The Participant is voluntarily participating in the Activities. The Parties acknowledge that participation in soccer involves inherent risks, dangers, and hazards, including but not limited to:

a) Injuries resulting from the sport of soccer or the nature of the activities undertaken;
b) Vigorous physical exertion, strenuous cardiovascular workouts, or rapid movements;
c) Exerting or stretching various muscle groups;
d) Grass, turf, gym floors, and other surfaces, including bacterial infections and rashes;
e) Falls, slips, or collisions with other participants, equipment, or structures;
f) Contact with balls, goals, or training equipment;
g) Extreme weather conditions (heatstroke, sunstroke, dehydration, or hypothermia);
h) Dry-land training, including running, weights, or circuit exercises;
i) Improper use or mechanical failure of equipment;
j) Spinal cord, head, or other serious injuries that may result in permanent impairment;
k) Travel to and from club events or activities.

4. Additional Acknowledgements

The Parties further understand and acknowledge that:

a) Injuries sustained while participating may be severe;
b) The Participant may experience anxiety or discomfort when performing challenging activities;
c) Close physical contact with others is inherent to the sport;
d) Following all rules and instructions reduces the risk of injury;
e) Fatigue may increase the likelihood of injury.

5. Release of Liability

In consideration of the Club allowing the Participant to take part in the Activities, the Parties agree:

a) That the Participant is physically and mentally fit to participate;
b) To freely accept and fully assume all risks, dangers, and hazards, including the possibility of personal injury, death, property damage, or loss resulting from participation; and
c) To forever release and discharge the Club from any and all actions, claims, demands, or costs that may arise out of the Participant’s participation in the Activities, including those arising from the negligence of the Club.

6. Conduct and Participation Policy

I understand that the Club reserves the right to refuse participation or remove any participant for behavior deemed unsafe, inappropriate, or disruptive, without refund. I also agree not to hold the Club or its representatives responsible in the event that my son/daughter/child engages in inappropriate conduct (including, but not limited to, disruptive or volatile behavior in or out of the program) or becomes involved in any activity or with any persons not associated with Northern Lions Youth Soccer Club or its scheduled program, and I acknowledge that the Club has the right to send him/her home for inappropriate conduct.

7. Media and Information Consent

I give permission for my child to be photographed or recorded for marketing or promotional purposes. I further attest that the information contained in this application is correct to the best of my knowledge. In addition, I have read and agreed to the Club’s policy and fee statement and agree to comply.

8. Acknowledgement

The Parties acknowledge that they have read and understood this agreement, agree to be bound by its terms, and understand that this agreement is binding upon themselves, their heirs, executors, administrators, and representatives.
